Evaluation of suspected Malignant Hyperthermia events during anaesthesia

چکیده
Background: Malignant hyperthermia (MH), a metabolic myopathy triggered by volatile anesthetics and depolarizing muscle relaxants, is a potentially lethal complication of general anesthesia in susceptible patients. The implementation of modern volatile anesthetics that pretend to be less potent trigger substances and the recommended limitations of succinylcholine use are suggesting a considerable decline of fulminant MH cases. Aim of this study was the evaluation of incidence and severity of suspected MH episodes during general anesthesia in patients that were referred to the Wuerzburg MH unit between 2007 and 2011. Methods: With IRB approval data of patients that underwent muscle biopsy and in vitro contracture test (IVCT) between 2007 and 2011 were analyzed. Only patients with a history of suspected MH crisis were included in the study. The incidents were evaluated retrospectively using anesthetic documentations and medical records. Results: Between 2007 and 2011 a total of 124 patients had been tested. 19 of them were referred because of suspected MH events; 7 were diagnosed MH-susceptible, 4 MH-equivocal and 8 MH-non-susceptible according to IVCT. In a majority masseter spasm after succinylcholine had been the primary symptom. Cardiac arrhythmia and hypercapnia frequently occurred early in the course of events. Interestingly, dantrolene treatment was initiated in a few cases only. Discussion: MH is still an important anesthetic complication. Every anesthetist must be aware of this lifethreatening syndrome at any time. The rapid onset of adequate therapy is crucial to avoid major harm and possibly lethal outcome. Dantrolene must be readily available wherever MH triggering agents are used for anesthesia.

Suspected recurrence of malignant hyperthermia after post-extubation shivering in the intensive care unit, 18 h after tonsillectomy.
A 25-yr-old man, subsequently shown to be malignant hyperthermia (MH) susceptible by in vitro contracture testing, developed MH during anaesthesia for tonsillectomy. Prompt treatment, including dantrolene, led to rapid resolution of the metabolic crisis. Eighteen hours later the patient's trachea was extubated in the ICU, when he had been stable and apyrexial overnight. Twenty minutes after ext...
متن کاملChanges in creatine phosphokinase (CK) concentrations after minor and major surgeries in children.
BACKGROUND During surgery, damage occurs to muscles in the area of the operation. The few studies that have examined creatine phosphokinase (CK) values after surgery have been in adults. The only study in children was after cardiac surgery. Understanding the normal enzyme pattern of change may help to differentiate malignant hyperthermia, anaesthesia-induced rhabdomyolysis and elevated CK value...
متن کاملDoes training on an anaesthesia simulator lead to improvement in performance?
We have used the Leiden anaesthesia simulator, which makes use of a standard anaesthesia machine and monitors, and realistically simulates the anaesthesia work place. After obtaining informed consent, 28 anaesthetists and anaesthesia trainees in one hospital took part in the study. All participants were exposed to a pre-scripted simulated "control" scenario of anaphylactic shock (phase 1). The ...

We report a case of acute hypermetabolism following the induction of general anaesthesia in an 11-yr-old boy. This episode was diagnosed and managed as an acute malignant hyperthermia crisis. However, severe hypertension during the episode led to the discovery of an unsuspected phaeochromocytoma. A hypermetabolic state during anaesthesia has several aetiologies, but correct diagnosis during the...

It is 30 yr since the British Journal of Anaesthesia published the first consensus protocol for the laboratory diagnosis of malignant hyperthermia susceptibility from the European Malignant Hyperthermia Group. This has subsequently been used in more than 10 000 individuals worldwide to inform use of anaesthetic drugs in these patients with increased risk of developing malignant hyperthermia dur...
